Select “My Downloadable Products” link
Select “Recertification by Professional Credit Plan” link
The PDF form is opened and is ready for completion. (NOTE: due to recent issues with Chrome that have not been resolved [and won’t be for another 6-weeks according to their blog], you must use Internet Explorer to open and save the PDF to your computer for use).
